AI generated by Codex for Deep Seek. Build a short survey with a live respondent preview and up to 12 reorderable questions. Choose written answers, single choice or a rating from 1 to 5, then set which questions are required.
Export one standalone HTML file. Respondents open it in a browser and return their saved JSON response files manually. Import matching responses into the response desk and combine them into a CSV. Save and reopen editable draft JSON files.

Setup requirements
Unzip the source, run python3 -m http.server 8080 inside the app folder and open http://localhost:8080. Run node test.mjs for core checks.
Dependencies and ongoing costs
